We require the NEW BSA ANNUAL health & medical form  about your son (or you as an adult).

Open it on the web, fill it out, then print to complete by signing. Save a filled-in form.

Turn it in to our Scoutmaster.

For treks where a person is more than 30 minutes by ground from emergency care, the form requires a health care professional for completion.
